ESSENTIAL QUALIFICATIONS (Knowledge, Skill, Experience)
-Membership/eligibility for membership in the Ontario College of Social Workers & Social Service Workers
-Bachelor's Degree in Social Sciences (Social Work, Psychology, Sociology)
-Masters in (Social Work, Psychology, or Sociology) preferred
-Previous clinical experience working with the geriatric mental health population, inclusive of Long Term Care facilities and young adults including such conditions as depression, dementia, mood/affect disorders
-Knowledge and experience in providing education and training using adult learning principles to care providers and other in a variety of settings including long-term care and community settings
-Knowledge and experience in best practices related to the management of mental illness, dementias and behavioural problems, including use of effective practice in therapeutic modalities inclusive of cognitive behavioural therapy.
-Advanced skills in geriatric mental health and early psychosis assessment, care planning and interventions
-Previous experience in community outreach work with both geriatric persons with mental health issues and younger persons with mental health issues inclusive of psychosis
-Experience in the provision of family, client, and community education on mental health including establishing and mentoring groups in mental health populations
-Experience in working with community resources for finances, placement, housing and recreational activities.
-Valid Ontario Class "G" driver's license and reliable vehicle required as clinical services are outreach/community-based.

ACCOUNTABILITIES (Major Responsibilities of the Position)
-Reports to the Clinical Manager of Mental Health Services.
-Provides Social Worker consultation, assessment, group and individual treatment and education to healthcare providers and care givers in LTC and other community settings inclusive of care centers where younger persons reside.
-Acts as a expert clinical resource for patients, families and other health professionals both within this facility and in the community
-Awareness of the Mental Health Act and mental health services in Chatham-Kent and surrounding areas
-Facilitates the development, implementation, evaluation and update of policies, standards, program planning, and processes of the Social Worker Mental Health services
-Works closely with inpatient and outpatient Mental Health Services to provide seamless services
