ESSENTIAL QUALIFICATIONS (Knowledge, Skill, Experience)
-Member in good standing with a current Ontario Certificate of Registration with the College of Nurses of Ontario.
-Minimum of 2 years recent clinical nursing experience.
-Previous experience working with a Geriatric, Rehabilitation, Continuing Care population and care team.
-Knowledge and skill of wound care and enteral Feeding.
-Knowledge of the philosophy, function and operation of the Rehab/Complex Continuing Care Program.
-Completion of Palliative Care Level 1 and CAPCE certification.
-Certification in Geriatrics from the Canadian Nurses Association is an asset.

ACCOUNTABILITIES (Major Responsibilities of the Position)
-Conduct pre-admission patient assessments for admission eligibility to CCC.
-Conduct comprehensive geriatric and general patient assessments on all admitted and discharged CCC patients.
-Utilize interdisciplinary, evidenced-based and experiential-based practices to provide quality patient care to a medically complex population.
-Provide clinical leadership and support to the Registered Practical Nursing staff members.
-Facilitate the development of clinical expertise of nursing staff and sharing of knowledge with interdisciplinary team members.
-Act as an expert clinical nursing resource for patients, families and other health professionals.
-Implement any patient care beyond the scope of practice for enhanced RPN staff members.
-Implement best clinical practice for CCC nursing care for inpatients through protocols, processes, standards and/or consistent .procedures to ensure excellence in care, utilization, and efficiency.
-Participate in clinical staff educational opportunities.
-Utilize research and continuous quality improvement techniques.
-Participate in the decision-making process as a team member of the Rehab/CCC Program.
