ESSENTIAL QUALIFICATIONS (Knowledge, Skill, Experience)
-Registration in a University Pharmacy Program
-Registration with the Ontario College of Pharmacists
-Experience with Ontario's Meds Check Program or other Med Rec experience an asset
-Familiarity with drug names, dosages and appearances is an asset
-Computer skills are an asset
-Satisfactory Attendance

ACCOUNTABILITIES (Major Responsibilities of the Position)
-Patient/Family Interview at Admission to document a complete and accurate Best Possible Medication History
-Discharge Medication Reconciliation, including: communication with prescriber, nurse, pharmacist, patient, family, primary care provider and drug store; preparation of Discharge Med Rec documents; updating eBPMH
-Investigation of various Med Rec issues as identified by the pharmacists
-Med Rec audit, other activities as required by Director of Pharmacy
